RESIDENT CARE MANAGER

Care Coordination & Oversight o Develop, update, and monitor individualized resident care plans in collaboration with the interdisciplinary team. o Ensure that care plans reflect the physical, emotional, and psychosocial needs of each resident. o Conduct routine assessments and communicate changes in condition to the physician and appropriate staff. • Resident Care & Advocacy o Monitor resident health status and provide direct nursing care as needed. o Advocate for residents’ rights and quality of life, ensuring dignity, respect, and safety. o Act as the primary point of contact for families regarding resident care updates. • Team Leadership & Supervision o Supervise and support CNAs and other nursing staff in the delivery of daily care.…
