LPN/RN Nurse Navigator – Palliative Care Phoenix Home Care & Hospice is seeking a compassionate and experienced LPN or RN Nurse Navigator to join our Comprehensive Disease Management (CDM) / Palliative Care team in Springfield, MO and surrounding communities. Schedule: Full Time | Monday–Friday | 8:00 AM–5:00 PM Location: Springfield, MO and surrounding areas Salary: $60,000–$70,000 per year About the Role The Palliative Care Nurse Navigator helps patients and families navigate complex healthcare needs while receiving support in their homes and communities.
