Position Summary As Care Coordinator, you are a single point of coordination for patients, caregivers & families, and healthcare providers when a Praxis therapy is prescribed, ensuring each has the clarity, support, and follow-through needed to initiate and maintain therapy. You are responsible for overseeing the management of patient cases and ensuring timely completion of related tasks throughout the full patient journey.
