Provides mobile assessment services for inpatient consult liaison team and contract facilities. Collaborates with providers and multidisciplinary team members. Communicates with physicians and treatment team to facilitate transitions of care, including hospital admissions both inside and outside Prisma Health. Manages and collaborates in discharge planning for patients and coordinates with agencies providing post hospital care. Maintains records and document all assessments. Manages statistical data.
