RN-Care Coordinator 0105 Patient Continuum of Care Schedule: M-F 8am -4:30pm About the Position An “RN Care Coordinator” coordinates patient transition along the healthcare continuum through assessments, referrals and interdisciplinary planning for the high risk patient population. The Care Coordinator focuses on the education, social needs and on-going interactions with the patient, in the acute, post-acute and outpatient setting. The Care Coordinator works closely with the patient, their support system, physicians, and outside support agencies; such as home health care (HHC) agencies, extended care facilities (ECF), home medical equipment (HME) agencies, etc.…
