Overview Works independently within the practice and serves as a practice liaison in interactions with other RMG departments to include the remote health care coach team, the Complex Case Managers, and other health care and community services. Focuses on preventive health care as it relates to annual wellness visits and closing care gaps. What you will do Utilizes Practice Dashboard to determine all Medicare eligible patients for Annual Wellness Visits and subsequent AWVs.
