Clinical Pharmacist Specialist will be primarily responsible for medication management of patients in ambulatory setting and for ensuring safe and effective medication use, including chemotherapy, chemotherapy orders review and verification, medication therapy management including symptom management, therapeutic drug monitoring, and patient and patient’s caregivers education.…
