Discharge Care Coordinator

Develop and monitor patient-centered plans and treatments goals Assure the deliverance of quality treatment to patients and their families Provide quality case management services to all patients and their families Serve as a member of interdisciplinary team supporting the organization's treatment program and philosophy. Assists with discharge planning, including meeting with patients/families to gather basic data and formulate an initial discharge plan. Coordinate services with other treatment team members. Assists with member discharge evaluations, works with teams to build treatment plans that promote long-term health and relapse prevention. Maintain effective quality clinical records.
