Job Description Summary The Clinical Transition Coach (CTC) assists in the safe and timely transition of patients from acute inpatient care to alternative levels of care, including but not limited to a home care program, palliative/hospice program, skilled nursing facility, or home setting appropriate to the patient’s needs.
