City/State Charlottesville, VA Work Shift First (Days) Overview: Sentara is hiring a Care Coordinator for our Population Health Ambulatory Clinical Support Team Location: Sentara Quality Care Network Charlottesville Status: Full Time (40 hours/week) Shift: Days (Anticipated 8AM-5PM) *This Hybrid position requires 2-3 days per week in the office, in addition to days on site at affiliate practices, and allows for some work from home days. The Care Coordinator performs a variety of casework duties and provides case management services to patients, families, and designated caregivers.
