About this role

Job Description: Conduct thorough evaluations of patients to assess mental health needs and substance use patterns to determine appropriate plan of care and treatment needs. Develop, implement, and modify personalized treatment plans tailored to the specific needs of each patient. Monitor for changes in status or goal completion. Provide counseling, using a variety of therapeutic modalities, to patients with co-occurring substance use and mental health diagnoses in group/individual sessions to assist the patient with successful transition a community-based treatment model. Provide psychoeducational to patients and their families about co-occurring disorders, considerations for treatment, strategies for symptom management, and available community-based resources.…
