Job Description: ESSENTIAL JOB FUNCTIONS 1. Conducts MPW assessment, including a face-to-face visit with the individual in the home within the 14 day deadline, or other deadline, as established by state criteria. 2. Assesses and documents the individual’s physical health, mental health, social supports, and environment, using assessment tool designated by the state. 3. Observes and interviews the individual and his or her representatives or family members to identify the needs and services that the recipient or recipient’s family cannot manage or arrange on the individual’s behalf. 4. Explains paperwork to the individual, and/or guardian, and has them complete the Freedom of Choice form, along with other state and agency required paperwork, at the face-to-face assessment visit. 5.…
