About this role
Do you have experience supporting older Australians and a passion for helping people navigate their aged care journey? As a Care Partner, you will work closely with clients receiving government-funded aged care services in the Southeast Sydney region and surrounds. You'll build meaningful relationships with clients, assess their needs, coordinate services and advocate for positive outcomes that help them remain safe, independent and connected in their own homes. This is a full-time permanent position, combining community-based client visits with the flexibility to work autonomously from home. What's on offer Full-time permanent position.
