Requirements Hier maak jij het verschil Het FACT-team behandelt volwassenen tot 65 jaar met een ernstige psychiatrische aandoening (EPA). Zij hebben hulpvragen op verschillende levensgebieden, zoals wonen, werken, zingeving, gezondheid, financiën en sociale contacten. We werken volgens de F-ACT-methode: een intensieve vorm van ambulante en wijkgerichte behandeling, waarbij verschillende disciplines vanuit een herstelgerichte visie samenwerken binnen één team.…
