About this role

Word trajectbegeleider in een FACT-team waar jouw blik écht telt. Je begeleidt cliënten bij het vinden van passende dagbesteding: een plek waar zij structuur, eigenwaarde en perspectief kunnen opbouwen. Dat doe je samen met een multidisciplinair team van specialisten, verpleegkundigen en begeleiders. Jij staat dicht bij de cliënt, ziet wat er speelt en signaleert wat nodig is en precies die observaties maken binnen het team het verschil. Requirements Passende zorg voor jouw cliënten Het FACT team behandelt ouderen (vanaf 65 jaar) met een ernstig psychiatrische aandoening (EPA) en hulpvragen op verschillende levensgebieden, zoals wonen, , dagbesteding,/ maatschappelijke participatie zingeving, gezondheid, zelfredzaamheid, financiën en sociale contacten.…
