Job Description The UW Medical Center-Montlake Social Work Department has an outstanding opportunity for a Social Work Assistant. WORK SCHEDULE Full-Time / 40 hours per week Day Shifts POSITION HIGHLIGHTS Partner with Social Workers (SW), Nurse Care Coordinators (NCCs), patients, their families and the health care team to address patients’ progression of care in inpatient and outpatient settings May assist the SW and NCC team in the coordination of the discharge planning process for patients, as well as ongoing coordination of resources and referrals in the outpatient settings This position reports to the Department of Social Work and Care Coordination.…
