Job Description UW Medical Center - Northwest has an outstanding opportunity for a Nurse Care Coordinator to join our Care Management team with the goal of achieving coordinated patient flow through an inpatient stay, culminating in a safe and timely discharge. This position works closely with all members of the interdisciplinary health care team, ensuring that the team is aware of the patient’s plan of care and that all disciplines are working toward the same goals for the patient.
