Position Summary To coordinate and facilitate patient care through the Valley Hospital Health Care System as it relates to a multidisciplinary approach to patient progression, discharge planning and utilization of resources. Education Master’s degree in social work from an accredited program. New Jersey Social Work License required. MSW and LSW or LCSW required. LCSW strongly preferred. Experience Minimum of three (3) years of experience as a Social Worker required. Skills Demonstrates effective interaction and communication (oral, writing, presenting) skills.
