Position Summary A Community Health Nurse is responsible for coordinating and providing personalized skilled nursing care to patients in their homes. This role involves assessing patients’ health needs, developing and updating individualized care plans, providing skilled clinical services such as wound care, drain management, catheter changes, medication management, and other nursing treatments.
