About this role

Position Summary The Patient Navigator provides individualized service, navigating and supporting patients throughout the health system. He/she works in a dynamic health care environment with multiple departments to ensure timely screening, diagnosis, treatment and supportive services. S/he works in conjunction with the Nurse Navigator and other health care professionals to evaluate and address the needs of patients served to ensure timely access to services needed and optimize health and wellness. Education Associates Degree preferred, Patient Navigator Certification preferred, or obtained within 1 year of employment Experience Basic understanding of medical terminology, health care systems and health care financing required.
