Location HAZELWOOD, MO 63042 Scheduled Hours 40 Position Summary Communicate with patients/clients daily regarding service needs, guidelines, and eligibility. Communicate with medical and supportive service providers/staff daily regarding scheduling, client/patient barriers, best practices, etc. Monitor Inventory, determine needs, and place orders for client service supplies (i.e. food pantry, toiletries, bus tickets, etc.) Track timeliness of services, services provided, report and address issues with services, trouble-shooting and resolution of issues. Complete service documentation in EPIC and additional client level databases. Monitor and request additional supplies from vendors and/or manager to ensure timely receipt and distribution.…
