Healthy Careers Start Here The Disease Management Health Educator is responsible for providing clinical support to members by encouraging them to become engaged in their health and facilitating the provider and member relationship. The incumbent must thoroughly understand Disease Management roles and responsibilities to facilitate a collaborative patient and provider relationship, which includes patient assessment, education, clinical plans of care documentation and adherence, and ongoing monitoring of patient health changes and compliance. Job-Specific Requirements : Bachelor’s degree in Nursing & RN licensure required. At least two years of healthcare clinical experience required.…
