Job Details Help transform the way care is delivered across our ambulatory practices. ChristianaCare is seeking a Practice Transformation Liaison to partner with primary care and ambulatory practices to improve workflows, strengthen quality outcomes and support the delivery of value-based care. We’re looking for a relationship builder, problem solver and change partner with strong customer service skills and a solid understanding of ambulatory or primary care operations.
