About this role

Clinical Pharmacist (MTM) Position Description: The Clinical Pharmacist is responsible for providing medication management and other clinical pharmacy services to optimize medication-related healthcare outcomes. The pharmacist primarily supports Medication Therapy Management (MTM) services and may provide additional clinical pharmacy services based on client and organizational needs. This part-time position is M-F, remote, and candidate must be available between 10-20 hours per week. Performance Objectives and Responsibilities: Duties may include but are not limited to: Provide MTM services, including Comprehensive Medication Reviews (CMRs), Targeted Medication Reviews (TMRs), adherence interventions, cost-effective interventions, and other medication-related clinical interventions.…
