About this role

The Good Seed is looking to place an Enhanced Care Management (ECM) Lead Care Manager. We are looking to hire in both Los Angeles and San Bernardino County. The ECM Lead Case Manager will assume responsibilities for community outreach and engagement. This position will determine eligibility, complete enrollment assessments, and perform outreach to potential ECM members to offer an enhanced case management program. A successful ECM Lead Care Manager understands the importance of empathy, advocacy, cultural competency, and follow-up assistance to help clients access the services needed to build and sustain healthy lives.
