About this role

We have an exciting opportunity for an independent, autonomous Prescribing Clinician to play a key role in our Care Coordination and Integrated Urgent Care services across the Bath, Swindon and Wiltshire locality on a permanent basis. We can support both part-time and full-time working arrangements on a fixed rota pattern basis. This role offers a unique blend of clinical assessment and system navigation, supporting patients to access the right care, in the right place, at the right time, helping to reduce unnecessary hospital admissions and improve patient outcomes. You will work across our Care Coordination service in hours and to support our Urgent Care service out of hours.…
