Job Description: Position Summary: The Home Health Clinical Team Assistant is responsible to coordinate and maintain scheduling for all disciplines. Support scheduling, running reports and other administrative tasks to allow the field staff and clinical manager to operate at the top of their licenses and titles. Additionally, will assist in the coordination of referrals, complete necessary screening of referrals, perform insurance authorizations, and assist with hospital discharge planning.
