About this role
Liquid Personnel is recruiting two Social Workers for its client’s Hospital Discharge Team in Stoke-on-Trent City. These roles are to backfill positions while existing team members are on secondment.
What will your responsibilities be?
Conduct assessments to identify patients’ social, emotional, and practical support needs
Develop safe and effective discharge plans in collaboration with the multidisciplinary team
Provide crisis intervention, emotional support, and counselling to patients and families
Advocate for patients to ensure access to appropriate services and community resources
Coordinate referrals to external agencies, housing services, mental health teams, and safeguarding teams…
