About this role

SUMMARY: PACE Physical Therapists evaluate and implement individualized treatment plans to improve mobility, restore function, maintain safety, and reduce pain. The Physical Therapist will collaborate with the Interdisciplinary Team to provide all-inclusive care for our participants so they can age safely in their homes. SPECIFIC DUTIES AND FUNCTIONS: Work in conjunction with the Interdisciplinary Team to coordinate rehabilitation services for participants at PACE Southeast Michigan. The Physical Therapist will perform assessments, provide treatment sessions, implement care plans, and order/issue Durable Medical Equipment. Document all care provided to participants in accordance with department standards and submit to medical records in a timely fashion.…
