Job Type: Regular Scheduled Hours: 40 Job Summary: Reports to the Social Work Manager, the Licensed Clinical Social Worker (LCSW) is a member of the Social Work Department located in the primary care offices to provide care management services to their patients. The LCSW is primarily responsible for face-to-face comprehensive biopsychosocial assessments, collaboration with patient’s primary care physician to determine diagnosis and treatment through brief therapy and continued follow-up with patients throughout the duration of the therapy process.
