Job Type: Regular Scheduled Hours: 40 Job Description: Experienced Geriatric APP to join PACE: Oversees community care for PACE enrollees. Completes CMS regulatory visits face to face visit requirements in collaboration with the attending physician. Provides clinical information to the inter disciplinary team as well as the physician to ensure appropriate care and collaboration. Pace coverage area is 8 counties in Northern Kentucky working with interdisciplinary team and medical director to provide care to promote highest functioning care possible. Reviews clinical information, coordinates and oversees patient care in consultation with the interdisciplinary team and other healthcare practitioners.…
