Case Manager FT | Highland Hospital | Charleston, West Virginia About the Job: PURPOSE STATEMENT: The Case Manager supports the clinical services department by providing case management to the different units within the facility. Responsible for the overall discharge resource connection and aftercare planning needs and placement of the patient based on their individual needs and as directed by the treatment team. Maintains routine contact with referral sources and providers while maintaining resource file. Participates in quality care monitoring for the department.
