Your Role as Care Partner As a Care Partner, you'll play a key role in helping older Australians remain independent, connected, and living well in their own homes. Working closely with customers, families, carers, and health professionals, you'll deliver person-centred care management that supports each customer's goals, preferences, and wellbeing. You'll deliver this role by: Developing, implementing, and reviewing personalised care plans that promote independence, wellbeing, and customer choice. Coordinating services and managing customer budgets within approved funding parameters. Building trusted relationships with customers, carers, families, and health professionals.
