Your Role as Case Manager – Youth & Health Education Service In this position you will provide case management and facilitation services to program clients accessing our service. Community outreach across the Gold Coast is also a component of this role. Key responsibilities of this position include: Provide client-centred case management, advocacy and support to help young people achieve their goals and address their individual needs. Develop, implement and review personalised support plans in collaboration with clients. Deliver youth-friendly support, including emotional wellbeing guidance, information and referrals to appropriate services. Facilitate educational programs, workshops and health promotion activities that meet the needs of the target group.…
