About this role
Be apart of bringing preventive and primary care directly to where patients are most comfortable their homes, community sites, skilled nursing facilities, and pop-up clinics. As a Field Nurse Practitioner, you’ll perform comprehensive assessments, manage acute and chronic conditions, coordinate care, and help reduce hospital readmissions. Opportunities available all of Washington State. Travel Requirements: This is a field-based role with local travel within your service area. Most visits are nearby, but some days may involve extended driving up to about 120–125 miles spread throughout the day (about 2–3 hours total drive time). Mileage reimbursement is provided.…
