Job Description UW Medical Center-Montlake has an outstanding opportunity for a Manager of Nurse Care Coordination . WORK SCHEDULE Full time / 100% FTE Primarily day shifts, Monday-Friday One week of on-call per month DEPARTMENT DESCRIPTION The Nurse Care Coordinator Manager oversees the staff and processes that support coordinated patient flow throughout the care continuum, including emergency services, pre-admission, inpatient stay, and post-acute care strategies. The department ensures all incoming patients are screened to identify potential discharge barriers and readmission risks, and ensures that all service teams are moving the patient forward to discharge, eliminating or limiting barriers throughout the stay.…
